Filling out your endodontist patient forms ahead of time helps us prepare for your appointment and reduces your time in the waiting room. What to Bring to Your Endodontic Appointment

To help your endodontic appointment preparation go smoothly, please bring the following items:

  • Valid photo ID (driver's license or government-issued ID)
  • Dental insurance card (if applicable)
  • Referral slip or notes from your general dentist
  • Any recent X-rays or records related to the tooth being treated
  • A list of current medications, including over-the-counter supplements
  • A list of known allergies

Root Canal Consultation: What to Expect

We understand that visiting an endodontist for the first time can feel overwhelming. Knowing what happens during your root canal consultation may help ease any worry you have about the appointment.

Before Your Procedure

Dr. Sonia Molina will review your dental history, examine the affected tooth, and take any necessary imaging — including 3D CT scans or digital X-rays — to get a clear picture of the issue. She will explain your diagnosis in simple terms, walk you through the recommended treatment plan, and answer every question you have. If root canal therapy is recommended, Dr. Molina will discuss the procedure step by step so there are no surprises.

If you are experiencing severe pain and need urgent care, we offer same-day dental emergency appointments to provide relief as quickly as possible.

Sedation & Comfort Options

Your comfort is a priority at every stage of treatment. We offer a full range of sedation and comfort options, including nitrous oxide, oral sedation, IV sedation, and general anesthesia administered by a board-certified dental anesthesiologist on staff. During your consultation, we will discuss which option may be right for you based on your comfort level and medical history.

Insurance & Payment Information

Molina Endodontics participates in an extensive network of PPO dental insurance plans. Accepted carriers include Aetna, Anthem, Blue Cross Blue Shield, Cigna, Delta Dental, Guardian, Humana, MetLife, Principal Financial, UnitedHealthcare, United Concordia, and many others. We also accept CareCredit financing for patients who prefer a flexible payment plan.

You can complete your patient forms online using our secure TruForm system. Forms are available in both English and Spanish. Simply click the appropriate link above, fill in your details, and submit before your appointment. This helps our team prepare so you spend less time in the waiting room.

Please bring a valid photo ID, your dental insurance card, a list of current medications, and any X-rays or referral notes from your general dentist. If you completed your patient forms online, there is nothing additional to fill out when you arrive.
